2018 Cards vs Cubs Raffle  // Official Rules and Regulations

1. Raffle tickets can be purchased for $50 each.
2. Contest open to Rotarians and non-Rotarians.
3. All legal residents of Arkansas 18 years old or older are eligible to enter.
4. The entry period is August 1, 2018, through August 26, 2018.
5. One winner will be chosen at random from all entries on August 26, 2018.
6. The winner will be notified by telephone, U.S. mail and/or e-­‐mail, and have 30 days to respond. If the prize is unclaimed after 30 days, the second place winner will be awarded the prize. Winner will be announced upon the signed return of the completed Prize Affidavit.
7. Odds of winning the prize depend upon the number of eligible entries.
8. No substitute prizes or cash equivalent shall be awarded.
9. Rotary Club of Bentonville reserves the right to cancel or modify the promotion if fraud, misconduct, or technical failures destroy the integrity of the raffle.
10. In the event of a termination of the promotion, a notice will be posted on www.bentonvillerotary.com
11. The prize is a trip for two to Chicago to see the St. Louis Cardinals play the Chicago Cubs September 29, 2018. The prize includes two (2) game tickets to the September 29, 2018 game between the St. Louis Cardinals and the Chicago Cubs, two (2) roundtrip airline tickets to Chicago, two (2) nights stay at the The Westin Michigan Avenue Chicago, and dinner for two (2) at the Berghoff Restaurant in Chicago, all representing a retail value of approximately $2,500. The winner is responsible for all other costs including meals, all transportation to and from the airport, hotel, baseball park, etc. and any other costs incurred during this trip.
12. Any and all taxes, e.g. federal, state, and local, are the sole responsibility of the contest winner, in accordance with all applicable laws and regulations. The winner will be required to provide his/her social security number for tax reporting purposes. The value of the prize will be reported to the winner on the applicable IRS form (W2 or 1099).
13. Winner must sign and return an Affidavit of Eligibility/Release of Liability/Prize Acceptance Form.
14. Acceptance of a prize constitutes permission to Rotary Club of Bentonville to use the name, photograph, voice, and/or likeness of the prize winner for advertising and publicity purposes without the prior approval of entrant and without compensation to the entrant.
15. By accepting a prize, winner agrees to hold Rotary Club of Bentonville (including without limitation its subsidiaries, affiliates, directors, officers, employees, and assigns) harmless against any and all claims and liability arising out of the use of the prize or relating in any way to prize or giveaway.
16. Winner assumes all liability for any injury or damage caused or claimed to be caused, by participation in this promotion or use or redemption of any prize.
17. By participating in this promotion, entrants agree to be bound by the Official Rules and Regulations.
18. Void outside of the United States and where prohibited by law.
